Safro Jobs partnered with a Hospice Care provider serving north, south, and west Philadelphia along with Germantown.

JOB SUMMARY:

Coordinate hospice care by working with other team members as well as with the patients/families to ensure needs are met.
Evaluate the effectiveness of hospice services to the patient and family on an ongoing basis.
Perform evaluations for hospice care along with informational visits.
Utilize the nursing process and best practices to provide professional care.
Coordinate patient care services with other members of the care team as well as contracted services when needed.
Update and document care plans and changes in the plan of care. Consult with the attending physician and Hospice Medical Director when needed.
Actively participate in QAPI meetings and activities as well as staff development meetings.

REQUIREMENTS:

Must be licensed RN in the state of Pennsylvania
1 year of hospice experience is preferred
Must be a licensed driver with an insured, reliable vehicle
