Overview
We are seeking a compassionate Home Health Nurse (RN or LPN) to support an established and growing Senior Care provider in Altoona, PA. This opportunity is available as either an 8-week contract or direct hire, depending on your preference.
In this role, you’ll provide both home-based and facility-based care to older adults in a highly autonomous, supportive environment. You’ll manage a small daily caseload of wellness visits while enjoying a consistent weekday schedule, no weekends, and mileage reimbursement.

Job Summary

Provide direct and indirect nursing care to participants ages 55–95

Conduct wellness visits (~30 minutes) including:

Physical assessments

Vital signs
-
Home and safety environment checks

Evaluation of current conditions

Medication compliance and patient education

Participate in morning team huddles at the care center

Travel to participant homes and affiliated facilities

Supervise licensed and non-licensed staff as appropriate

Collaborate with the Program Director, physicians, and interdisciplinary team

Maintain compliance with all applicable regulations and policies

Build strong, trusting relationships with participants and families
Typical Day
Start your day at the center for a brief morning meeting, gather supplies, then travel to participant homes and care facilities. You’ll complete wellness checks, review care plans, and help ensure participants remain safe, healthy, and independent.
Qualifications

Active RN or LPN license in the State of Pennsylvania

Valid driver’s license and reliable vehicle

Experience working with older adults preferred
